ABSTRACT

Objective The three-dimensional (3D) solid model of medulla oblongata-upper cervical spinal cord based on specimen pathological section data was established, and the stress and strain levels of medulla oblongata-upper cervical spinal cord under dentate process compression were obtained by finite element analysis, so as to provide references for clinical research. Methods Mimics was used to process the slice data, so as to establish the point cloud model. SolidWorks was used to locate, edit and optimize the point cloud model, so as to establish the 3D solid model. HyperMesh was used to establish the finite element model and ANSYS was used for finite element analysis. Results The medulla oblongata-upper cervical spinal cord model with clear boundary between gray matter and white matter and white matter fiber bundle was established. The stress and strain levels and stress-strain curves of white matter and gray matter under different compression degrees were obtained. Conclusions Combined with pathological sections of specimens and reverse engineering, the 3D medulla oblongata-upper cervical spinal cord model with clear morphology and structure of gray/white matter can be established. When the medulla oblongata-upper cervical spinal cord is compressed, the stress level of gray matter is lower than that of white matter, and about 20% of compression is the critical state of white matter. When the disease develops beyond the critical state, the biomechanical properties of white matter may fail, resulting in gray matter damage.

Point-of-care ultrasound(POCUS)was dominated by emergency or intensive medical doctors.POCUS was a technology which was used to explain the pathophysiological changes, diagnosis and treatments.POCUS was widely used in the evaluation of hemodynamics, lung diseases and the airway managements.In pediatric field, especially the critically newborn treatment, POCUS has brightly prospects.The application progresses of POCUS in neonatal intensive care unit were reviewed in this article.

ABSTRACT

Objective@#To explore hematological and genotypic characteristics of patients with hemoglobin E (Hb E) disorders from Yunnan Province.@*Methods@#One hundred individuals with Hb E disorders indicated by high performance liquid chromatography (HPLC) were subjected to genetic testing through multiple gap-PCR and reverse dot-blotting analysis.@*Results@#All patients were found to harbor a mutation to the 26th codon of the β-globin chain (HBB: c. 79G>A). Ninety patients were heterozygotes, and 10 co-inherited c. 79G>A and an α-thalassemia mutation (7 αα/-α3.7, 2 αα/--SEA and 1 -α3.7/-α3.7). Hematological characteristics of the heterozygotes were: Hb A2 (26.02±3.64)%, Hb F(1.35±1.25)%, MCV(78.83±4.68) fl, MCH(26±1.54) pg, MCHC (329.65±10.73) g/L, HGB (141.08±16.53) g/L, while that of the co-inherited cases was decided by the type of α-thalassemia mutation.@*Conclusion@#Hb E can be effectively detected by HPLC. The type of α-thalassemia mutations will determine hematological features of co-inherited cases. Hb E disorders may be missed by relying only on routine blood test upon prenatal screening.

ABSTRACT

OBJECTIVE:To study the improvem ent effects of total flavonoids from Morus alba on glycolipid metabolism , inflammation and oxidative stress in gestational diabetes mellitus (GDM)model rats ,and to investigate the potential mechanism. METHODS:After feeding high fat diet for 8 weeks,female SD rats with FBG <6.67 mmol/L were caged with male SD rats . Pregnant female rats were randomly divided into control group ,GDM group ,M. alba total flavonoids low-dose ,medium-dose and high-dose groups (50,100,200 mg/kg),with 10 rats in each group. Except for control group ,other groups were given intraperitoneal injection of streptozotocin 25 mg/kg once to induce GDM model. After injection ,rats in each administration group were given corresponding drugs intragastrically ,control group and GDM group were given normal saline 10 mL/kg intragastrically , once a day ,for consecutive 18 days. The levels of FBG were determined on the 3rd,7th and 18th day of pregnancy (G3d,G7d and G18 d);the levels of blood lipids (TG,TC,LDL-C,HDL-C)and inflammatory factors (TNF-α,IL -6,IL-8),oxidative stress indicators(MDA,SOD,GSH,CAT)in serum were determined on G 18d. The protein and mRNA expressions of PPARγ and NF-κB, the expression of AMPK mRNA and p-AMPK protein were measured by Real-time-PCR and Western blotting. RESULTS : Compared with control group ,the levels of FBG (G3d,G7d,G18d),TG,TC,LDL-C,TNF-α,IL-6,IL-8 and MDA ,protein and mRNA expression of NF-κB in GDM group were significantly increased,while the levels of SOD ,GSH and CAT ,the expressions of PPARγ protein and mRNA,AMPK mRNA and p-AMPK fcksjf@126.com protein were significantly decreased (P<0.01). Compared with GDM group ,the levels of FBG (G3 d,G7 d,G18 d),TG, huawei99@163.com TC,LDL-C in M. alba total flavonoids medium-dose and high- dose groups and the levels of TNF-α,IL-6,IL-8 and MDA ,protein and mRNA expression of NF-κB in M. alba total flavonoids groups were significantly decreased ;the levels of SOD ,GSH and CAT ,the expressions of PPARγ protein and mRNA, AMPK mRNA and p-AMPK protein were significantly increased (P<0.05 or P<0.01). CONCLUSIONS :Total flavonoids from M. alba can improve the glycolipid metablism ,inflammation and oxidative stress in GDM model rats ,the mechanism of which may be related to the activation of PPARγ pathway.

ABSTRACT

Disorders of cerebral hemodynamics are closely related to neonatal brain injury. Early, fast and accurate monitoring is of great significance for maintaining the stability of neonatal cerebral hemodynamics. As a common cerebral hemodynamic monitoring technique, color Doppler ultrasonography could assist with reducing neonatal long-term nervous system diseases and improving the quality of life. We review the progress in neonatal cerebral hemodynamic monitoring to provide reference for clinical practice.

ABSTRACT

Objective To explore the effect of nurse-social worker-college student volunteer team guide on care-giving competence of the primary caregivers for the long-term bedridden elderly. Methods The research was conveniently focused on a total of 60 long-term bedridden elderly and 30 primary caregivers, pension caregiver in welfare home from March to May 2015. The nurse-social worker-college student volunteer team guide for the primary caregivers included distributing health manual, training skill, psychological support, respite care and so on. The intervention lasted for 3 months, twice a month, 150 minutes each time. This was a self comparison study. The care competence for the primary caregivers and self-care ability of daily activities for the long-term bedridden elderly were conducted before and after intervention. Results Before intervention, the total score of care-giving competence was 59.07 ± 13.42. After intervention, the total score was 66.64 ± 14.16. Before intervention, the scores of caring knowledge, operation technique, behavior and attitudes, decision-making and self-efficacy were as follows:18.48±3.46, 17.95±3.98, 12.16±3.08, 7.98±2.56. Whereas, after intervention, the scores were as follows:20.32±3.58, 20.48±2.74, 14.29±3.03, 9.65±2.07. Each dimension score after intervention was significantly higher than that before intervention (t=-2.87--2.02, P<0.01). For bathing, dressing, indoor transferring and eating in self-care ability of daily activities, the numbers of people in 60 long-term bedridden elderly were 8, 13, 21, 18 before intervention and 17, 24, 43, 29 after intervention. The self-care ability of daily activities of the primary caregivers such as bathing, dressing, indoor transferring and eating after intervention were significantly higher than that before intervention (χ2=4.093-4.857, P<0.05). Conclusions Nurse-social worker-college student volunteer team guide can enhance the care-giving competence of the primary caregivers as well as improve the self-care ability of daily activities for the long-term bedridden elderly.

ABSTRACT

ObjectiveTo determine the optimal timing of antenatal taurine supplementation to improve neuron and neural stem cell proliferation in fetal rats with intrauterine growth restriction.Methods Twenty-five pregnant SD rats were randomly divided into five groups (five rats in each group): group A was the control group, group B to E were the fetal growth restriction (FGR) model groups with low-protein diet during the experiment, group C, D, and E were supplemented with taurine [300 mg/(kg·d)] at day 9, 11 and 15, respectively. The birth weight of newborn rats was measured after natural delivery. The rats with body weight two standard deviations lower than the average weight in group A were diagnosed as FGR. There were five litters of newborn rats in each group, and two were randomly selected from each litter, resulting in ten newborn rats in each group. Proliferating cell nuclear antigen (PCNA) and fatty acid binding protein 7 (FABP7) positive cell expression in newborn rat brain tissues were detected by immunohistochemistry. Single factor analysis of variance, LSD tests were used for statistical analysis.ResultsThe average birth weight of newborn rats in group A, B, C, D and E were (6.61±0.45), (4.65±0.23), (5.37±0.17), (5.74±0.21), and (5.00±0.24) g, respectively. Average birth weight was lower in group B than in group A (t=2.447), higher in group D and E than in group B (t=2.306 and 2.306), higher in group D than in group C and E (t=2.306 and 2.306), and the differences were statistically significant (allP0.05). The IOD in group D was higher than that in group E, and the difference was statistically significant (t=4.182,P<0.05).ConclusionsAntenatal taurine supplementation can promote neuron and neural stem cell proliferation in rats with FGR. The effect is most obvious on the 11th day of pregnancy, and may lead to the promotion of brain development.

ABSTRACT

Objective To evaluate whether or not the 3.0T magnetic resonance (MR) scanner noise has adverse influence on neonatal hearing by using brainstem auditory evoked potentials(BAEP).Methods Forty-nine inpatients who received the MR examination were enrolled in this study from Aug.to Dec.2013,admitted to the Bayi Children's Hospital Affiliated to Beijing Military General Hospital.The Ⅰ,Ⅲ,Ⅴ wave latencies and the inter-peak intervals before and after the MR scan were compared by using SPSS 16.0 software.Results The BAEP results before and after MR examination respectively:(1) The latencies were:left ear Ⅰ wave (1.96 ± 0.22) ms vs (1.95 ± 0.30) ms,right ear Ⅰ wave (1.96 ± 0.22) ms vs (1.97 ± 0.27) ms,respectively; left ear Ⅲ wave (4.79 ± 0.23) ms vs (4.85 ± 0.28) ms,right earlllwave(4.78 ±0.24) ms vs (4.77 ±0.31) ms,respectively;left ear Ⅴ wave (7.10 ±0.24) ms vs (7.12 ±0.33) ms,right ear Ⅴ wave (6.76 ±0.32) ms vs (7.04 ±0.39) ms,respectively(allP >0.05).(2) The inter-peak intervals were:left ear Ⅰ-Ⅲ inter-peak interval (2.83 ± 0.23) ms vs (2.86 ± 0.27) ms,right ear Ⅰ-Ⅲ inter-peak interval (2.82 ± 0.24) ms vs (2.80 ± 0.17) ms,respectively ; left ear Ⅲ-Ⅴ inter-peak interval (2.31 ±0.28) ms vs (2.31 ±0.29) ms,right ear Ⅲ-Ⅴ inter-peak interval (2.26 ±0.27) ms vs (2.26 ±0.23) ms,respectively;left ear Ⅰ-Ⅴ inter-peak interval (5.11 ±0.40) ms vs (5.13 ±0.35) ms,right ear Ⅰ-Ⅴ inter-peak interval (5.07 ± 0.39) ms vs (5.07 ± 0.36) ms,respectively(all P > 0.05).Conclusion The 3.0T MR may have no adverse influence on neonatal BAEP.

ABSTRACT

Objective To investigate the effect of kangaroo mother care on children′s comfort with intramuscular injection. Methods One hundred children with an intramuscular injection were grouped according to the injection date. Those treated with injection on odd-numbers days were assigned as the intervention group , and those with injection on even-numbered days as the control group, with 50 cases in each group. The control group received routine care and the intervention group with kangaroo mother care. The two groups were compared in terms of comfort level. Result The comfort level of the observation group was higher than that of the control group with statistical significance (P<0.01). Conclusion Kangaroo mother care can improve the comfort level of children patients with intramuscular injection. It is a simple, safe and effective in non-pharmaceutical pain therapy.

ABSTRACT

Objective To explore the effects of prenatal taurine supplementation on the expression of protein kinase A(PKA) and c-fos,Ca2+/calmodulin-dependent protein kinase Ⅱ (GaMK Ⅱ) in fetal rat brains with intrauterine growth restriction and its significance.Methods Fifteen pregnant Sprague-Dawley rats were randomly divided into 3 groups:the control group,the intrauterine growth restriction (IUGR) group and the IUGR with prenatal taurine supplementation group(the taurine group),with 5 fetal rats in each group.All the fifteen fetal rat brains were detected as following:the expression of PKA,c-fos,CaMK Ⅱ mRNA in fetal rat brains was detected by way of real-time polymerase chain reaction(real-time PCR),while the expression changes of PKA,CaMK Ⅱ,and c-fos protein in fetal rat brains were detected by using Western blot,and the number of PKA,CaMK Ⅱ,c-fos positive cells in fetal rat brains was detected by using immunohistochemistry.Results The control group,the IUGR group and the taurine group:Comparison of the expression of PKA,CaMK Ⅱ,c-fos mRNA among 3 groups were of significant differences(F =7.934,P =0.021 ; F =5.568,P =0.043 ;F =7.332,P =0.024).Comparison of the expression of PKA,CaMK Ⅱ,c-fos protein among the 3 groups were of significant differences(F =57.743,P =0.000 ; F =163.405,P =0.000 ; F =160.136,P =0.000).Comparison of the number of PKA,CaMK Ⅱ,c-fos positive cells among the 3 groups were of significant differences (F =42.903,P =0.000 ;F =329.123,P =0.000 ; F =43.674,P =0.000).Compared with the control group,the expression of mRNA,protein,positive cells of three indicators in IUGR group was less,and the difference was statistically significant (P < 0.05).The expression of mRNA,protein,positive cells of three indicators in fetal rat brains of the taurine group were not different from the control group.The expression of mRNA,protein,positive cells of three indicators in fetal rat brains of the taurine group were significantly more than the IUGR group(P < 0.05).Conclusions Prenatal taurine supplementation can improve PKA-CaMK Ⅱ,c-fos mRNA and protein levels in fetal rat brain tissue with IUGR,and increase its number of positive cells and may enhance the regenerative capacity of the central nervous system,so as to reduce the IUGR brain injury and promote its role in brain development.

Diffusion tensor imaging is currently the best noninvasive technique to assess microstmctural changes in white matter pathways,which enables quantitative assessment of brain normal structures,brain development and lesions by calculating the apparent diffusion coefficient,fractional anisotropy,relative anisotropy and parallel diffusivity,etc.In order to improve the study on neonatal brain injuries,the application of diffusion tensor imaging in neonatal fields was reviewed in this paper.

ABSTRACT

Objective To evaluate the value of lung ultrasound in diagnosing infectious pneumonia (IPN) of newborns. Methods Seventy-two infants hospitalized from September 1, 2012 to May 30, 2013, in Bayi Children's Hospital, General Hospital of Beijing Military Command were divided into two groups. The study group consisted of 32 newborn infants diagnosed as IPN by medical history, clinical manifestations and chest X-ray, while the control group consisted of 40 neonates without any lung diseases. In a quiet state, the infants were placed in supine, side or prone position for lung ultrasound examination. The lung field was divided into three areas by the anterior and posterior axillary line. The regions of the bilateral lung were scanned by the probe which was vertical with the ribs. Fisher's exact was performed for stastical analysis. Results The most important ultrasound imaging findings of IPN included lung consolidation of varying size and shape with irregular and serrated margins (100%, 32/32), dynamic air bronchograms (100%, 32/32), A-line disappearance (100%, 32/32) and interstitial syndrome (100%, 32/32). Other common ultrasound signs included pleural line abnormalities (91%, 29/32), lung pulse (38%, 12/32) and dynamic air bronchograms (66%, 21/32) under real-time ultrasound. Pleural effusion was an infrequent sign that existed in 9%(3/32)of IPN patients. None of these abnormalities was observed in the control group. Conclusion Lung ultrasound is reliable for the diagnosis of IPN and can be routinely performed in neonatal intensive care units, and a large area of lung consolidation with irregular margins A-line disappearance, interstitial syndrome and pleural line abnormalities are the main image findings.

ABSTRACT

Objective To determine the adverse effects of fetal growth restriction (FGR) on the development of fetal cerebral function.Methods Sixty small for gestational age (SGA) and 40 appropriate gestational age (AGA) infants with a gestational age ranging from 37 weeks to 40+6 weeks who were hospitalized in the Ba Yi Children's Hospital Affiliated to Beijing Military General Hospital were included in this study.Patients with fetal distress,birth asphyxia,hyperspasmia,intracranial hemorrhage,periventricular leukomalacia,intracranial infection,septicemia and congenital abnormalities were excluded from the study.Amplitude-integrated electroencephalography (aEEG) was used to monitor the baby's cerebral function within three days after birth.The monitoring time was two to four hours in each infant.The observational indices included continuity,sleep wake cycling (SWC),interburst interval (IBI),minimum voltage and maximum voltage.Student's t test,Chi-square test and analysis of variance were used to compare differences between the groups.Results Continuity:the frequency ofaEEG continuity in the SGA and AGA groups was 15.0% (9/60) and 70.0% (28/40),respectively (x2=31.14,P=0.00).SWC:the frequency of aEEG SWC in the SGA and AGA groups was 15.0% (9/60) and 70.0% (28/40) (x2=31.14,P-0.00).IBI:the frequency of IBI in the SGAandAGAgroups was (14.55±0.75) s and (5.09±0.89) s (t=57.21,P=0.00).Maximum voltage:the maximum voltage in the SGA and AGA groups was (10.40± 2.61) t V and (16.42± 5.53) μ V (t=-6.43,P=0.00).Minimum voltage:the minimum voltage in the SGA and AGA groups was (4.02± 1.61) μ V and (6.98±3.82) μV (t=4.63,P=0.00).Lower birth weight,hypoglycemia (without hypoglycemic encephalopathy) or hyperbilirubinemia (without bilirubin encephalopathy) had no significant influence on fetal cerebral function (all P>0.05).Conclusions SGA infants have poor continuity,absence of SWC,longer IBI,and lower maximum and minimum voltage,which suggests that FGR,to an extent,has an adverse influence on the development of fetal cerebral function.

[Objective]Study on the Pathology shape structure of calcified cartilage one in Osteoarthritis knee joint and provide the theory for diagnosis and treatment of correlated disease.[Method]Osteoarthritis condyles of femur(n=21) were collected from knee joint replacement and the paraffin sections were prepared after fixation and decalcification.After stained with Safranin O/fast green,the shape structure of calcified cartilage zone was observed by microscope.[Result]Mean age of osteoarthritis patient was 65.57?7.43 yearsold;mean stature was(155.38+5.32)cm;mean weight was(59.95 ?8.99)kg;mean pathogenesis was(13.04?9.66) years.After stained with Safranin O/fast green,the pathological changes of calcified cartilage zone were defined as follow.Tidemark drift and thickening accompany with blood vessel invasion.The gap of tidemark increased width.Calcified cartilage zone desmoid changed or defect.[Conclusion]Calcified cartilage zone in Osteoarthritis knee joint taken place specific pathological changes.
